Case information
Dental practices should submit only information necessary for the laboratory service and should use a patient ID or case reference instead of a full patient name whenever practical. The prescribing dental professional is responsible for ensuring that they are authorized to provide case information and records to the laboratory.
Email transmission
Website forms on this version of the site route submissions to the laboratory by email. Standard email is not end-to-end encrypted. Do not submit information that is unnecessary for the laboratory case. Contact us to arrange another transfer method for large or particularly sensitive datasets.
How we use information
Information is used to respond to inquiries, prepare or discuss laboratory services, process dentist-authorized laboratory cases, provide requested pricing information, coordinate pickup, and maintain necessary business records.
Sharing
We do not sell submitted information. Information may be handled by service providers involved in website hosting, email delivery or business operations where reasonably necessary.
